Intake Interview

A critical step in planning test anxiety services is to determine the specific needs of the individual. Test Anxiety is a very subjective experience, and not all students need the same type of assistance. The intake invertiew included here (in Word Format) is the one used by Dr. Ross when he interviews students to develop a plan of action. It is very helpful to determine if the student needs help with: study strategies, test taking, diet modification, attitudes, relaxation or desensitization. Also included here is a short test anxiety scale that was developed by Dr. Richard Driscoll of the American Test Anxieties Association.

School personnel are authorized to copy the materials for use in a school setting, provided proper credit is given.
